In PowerPC (pseries) a non-virtualized adapter will have 2 DMA windows -
2GB default and a larger Dynamic DMA Window (DDW). DDW is large enough to
map total RAM to a device.
During normal functioning of OS, since RAM is pre-mapped, 2GB default
window is not used. The only scenario it might get used is when buffers in
pmemory are mapped to the device for DMA.
As of today, during kdump, during early device discovery, pci_dma_find()
finds that the device has 2 DMA windows. It selects to use DDW. This is a
kdump path and DMA window is needed for IO to the device.
Although commit 09a3c1e46142 ("powerpc/pseries/iommu: IOMMU table is not
initialized for kdump over SR-IOV") fixed an issue during kdump with SR-IOV
case, but this also made the kdump prefer DDW over the default DMA window
when both are present (dedicated adapter case). Since the DDW is fully
mapped by the previous kernel, iommu_table_clear() can free only
KDUMP_MIN_TCE_ENTRIES (2048) TCEs for use by kdump kernel.
This is not enough when the dump device is NVMe over Fibre Channel.
Because nvme-fc driver DMA-maps the cmds and resp IUs of every
pre-allocated request and each such mapping consumes roughly:
32 (IO queues, one per cpus = nr_cpus) *
64 (queue_depth, blk-mq kdump limit) *
2 (cmd+resp) = 4096
This is already double of what we have without counting admin queues and
lpfc driver's own allocations / mapping requirement. Hence this results
into iommu_alloc failures like -
lpfc 0153:70:00.0: iommu_alloc failed,
tbl 0000000034ebcf5e vaddr 00000000d814df0b npages 1

Increasing the number of free TCE entries in iommu_table_clear() will
increase the probability of hitting EEH since there could still be some
active IOs from the previous life of the kernel.
Hence this patch partially reverts the previous fixes commit and
switches the kdump's default back to 2GB default DMA window instead of
DDW window. This window will mostly be empty. Or, could be slightly used
if buffers in pmemory were mapped for IO.

On pseries LPAR systems, watchdog timers configured from userspace can
remain active after a kernel panic. When a panic triggers kdump, the
crashing kernel jumps directly to the kdump kernel without stopping
active watchdogs. As a result, the watchdogs remain active after the
kdump kernel starts.
If dump capture takes longer than the watchdog timeout, PHYP resets the
LPAR before the dump is fully captured, causing dump capture to fail.
Fix this by issuing the `H_WATCHDOG` hcall during the crash shutdown
sequence to stop all active watchdogs before booting the kdump kernel.

The pseries watchdog initialization registers the pseries-wdt platform
device using platform_device_register_simple(), but currently ignores
its return value.
Check the returned pointer for errors, log a descriptive error message
when registration fails, and propagate the failure code to the caller.
This avoids silently ignoring platform device registration failures.

When xive_init_ipis() fails (e.g. irq_domain_alloc_irqs() fails),
the error path frees the global xive_ipis array. However,
xive_smp_probe() previously ignored this failure and proceeded to
call xive_setup_cpu_ipi(), which dereferences the already-freed
xive_ipis pointer -- a use-after-free.
Now that xive_smp_probe() returns int (previous patch), propagate
the error from xive_init_ipis() and xive_setup_cpu_ipi() through
xive_smp_probe(). Check the return value in both pnv_smp_probe()
and pSeries_smp_probe() so that IPI setup is aborted cleanly on
failure, avoiding the use-after-free.

arm64 Realms, powerpc secure guests and s390 protected virtualization
guests currently report CC_ATTR_MEM_ENCRYPT, but not
CC_ATTR_GUEST_MEM_ENCRYPT.
These environments are all secure guest configurations. Their
force_dma_unencrypted() implementations also treat the same secure guest
state as requiring unencrypted DMA. Report CC_ATTR_GUEST_MEM_ENCRYPT for
the same condition so generic confidential-computing code can distinguish

During pSeries_setup_arch(), VPA for boot-cpu is first to be
initialized. However later in the boot, smp_setup_cpu() is called for
setting up VPA on boot and secondary cpus that were brought online. This
results in vpa_init() being called twice for boot-cpu and three redundant
H_REGISTER_VPA hcalls being made to the hypervisor.
Fix this by adding an extra condition in smp_set_cpu() to call vpa_init()
only on non boot-cpus.

Currently pseries_kexec_cpu_down() skips unregistering vpa, slb_shadow and
dtl areas during a crash and kexec shutdown path. It was done to avoid
doing an HCALL while crashing. However recently Anushree reported that
during kernel crash while the kdump kernel was coming up, Hypervisor
reported invalid values for 'vpa.yield_count' while it dispatching L2-KVM
Guest vcpus. The error manifested as debug build Hypervisor assert
triggering to indicate possible VPA corruption.
Looking at the kexec cpu offline path it was discovered that during crash
kernel doesn't unregister the VPA/SLB-Shadow/DTL area with
Hypervisor. Instead it re-allocates and re-registers these areas
for cpus during boot. During kexec boot the previously allocated areas
can get overwritten with new content without hypervisor knowledge. This
creates a small window where while kexec kernel boots and the L2-VCPUs are
being dispatched, Hypervisor may try to read/write to a wrong memory area
which previously belonged to older VPA.
Fix this possible race and memory corruption by updating
pseries_kexec_cpu_down() to also unregister vpa,slb_shadow & dtl areas
during a kernel crash.

git://git.kernel.org/pub/scm/linux/kernel/git/akpm/mm
Pull MM updates from Andrew Morton:
- "selftests/mm: clean up build output and verbosity" (Li Wang)
Remove some noise from the MM selftests build
- "mm: Free contiguous order-0 pages efficiently" (Ryan Roberts)
Speed up the freeing of a batch of 0-order pages by first scanning
them for coalescing opportunities. This is applicable to vfree() and
to the releasing of frozen pages
- "mm/damon: introduce DAMOS failed region quota charge ratio"
(SeongJae Park)
Address a DAMOS usability issue: The DAMOS quota often exhausts
prematurely because it charges for all memory attempted, causing slow
and inconsistent performance when actions fail on unreclaimable
memory.
To fix this, a new feature lets users set a smaller, flexible quota
charge ratio (via a numerator and denominator) for failed regions.
Since failed actions cause less overhead, reducing their quota cost
ensures more predictable and efficient DAMOS processing
- "selftests/cgroup: improve zswap tests robustness and support large
page sizes" (Li Wang)
Fix various spurious failures and improves the overall robustness of
the cgroup zswap selftests
- "fix MAP_DROPPABLE not supported errno" (Anthony Yznaga)
Fix an issue in the mlock selftests on arm32
- "mm: huge_memory: clean up defrag sysfs with shared" (Breno Leitao)
Some maintenance work in the huge_memory code
- "treewide: fixup gfp_t printks" (Brendan Jackman)
Use the special vprintf() gfp_t conversion in various places
- "mm: Fix vmemmap optimization accounting and initialization" (Muchun
Song)
Fix several bugs in the vmemmap optimization, mainly around incorrect
page accounting and memmap initialization in the DAX and memory
hotplug paths. It also fixes pageblock migratetype initialization and
struct page initialization for ZONE_DEVICE compound pages
- "mm/damon: repost non-hotfix reviewed patches in damon/next tree"
A sprinkle of unrelated minor bugfixes for DAMON
- "mm: remove page_mapped()" (David Hildenbrand)

Allow DAMON to be paused and resumed without losing its current state
- "kasan: hw_tags: Disable tagging for stack and page-tables" (Muhammad
Usama Anjum)
Simplify and speed up kasan by removing its ineffective tagging of
stacks and page tables
- "mm/damon/reclaim,lru_sort: monitor all system rams by default"
(SeongJae Park)
Simplify deployment on diverse hardware like NUMA systems by updating
DAMON_RECLAIM and DAMON_LRU_SORT to automatically monitor the
physical address range covering all System RAM areas by default,
replacing the overly restrictive behavior that only targeted the
single largest memory block to save on negligible overhead
- "mm/damon/sysfs: document filters/ directory as deprecated" (SeongJae
Park)
Update some DAMON docs
- "mm: use spinlock guards for zone lock" (Dmitry Ilvokhin)
Switch zone->lock handling over to using the guard() mechanisms
- "mm/filemap: tighten mmap_miss hit accounting" (fujunjie)
Fix a flaw where the mmap_miss counter over-credited page cache hits
during fault-arounds and page-fault retries. This results in
significant reduction of redundant synchronous mmap readahead I/O,
drastically cutting down execution time and gigabytes read for sparse
random or strided memory access workloads
- "selftests/cgroup: Fix false positive failures in test_percpu_basic"
(Li Wang)
Fix a couple of false-positives in the cgroup kmem selftests

Change DAMON_STAT to provide the pid of its kdamond
- "mm/kmemleak: dedupe verbose scan output" (Breno Leitao)
Remove large amounts of duplicated backtraces from the verbose-mode
kmemleak output
- "mm: remove CONFIG_HAVE_BOOTMEM_INFO_NODE (Part 1)" (David
Hildenbrand)
Reduce our use of CONFIG_HAVE_BOOTMEM_INFO_NODE, with a view to
removing it entirely in a later series
- "mm/damon: validate min_region_size to be power of 2" (Liew Rui Yan)
Prevent users from passing a non-power-of-2 value of `addr_unit', as
this later results in undesirable behavior

- "tools/mm/page-types: Fix misc bugs" (Ye Liu)
Fix three issues in tools/mm/page-types.c
- "mm: misc cleanups from __GFP_UNMAPPED series" (Brendan Jackman)
Implement several cleanups in the page allocator and related code
- "mm, swap: swap table phase IV: unify allocation" (Kairui Song)
Unify the allocation and charging of anon and shmem swap in folios,
provides better synchronization, consolidates the metadata
management, hence dropping the static array and map, and improves
performance
- "mm/damon: introduce data attributes monitoring" (SeongJae Park(
Extend DAMON to monitor general data attributes other than accesses
- "mm/vmalloc: free unused pages on vrealloc() shrink" (Shivam Kalra)
Implement the TODO in vrealloc() to unmap and free unused pages when
shrinking across a page boundary
- "mm/damon: documentation and comment fixes" (niecheng)
- "remove mmap_action success, error hooks" (Lorenzo Stoakes)
Eliminate custom hooks from mmap_action by removing the problematic
success_hook which allowed drivers to improperly access uninitialized
VMAs. It replaces the error_hook with a simple error-code field and
updates the memory char driver accordingly
- "mm/damon: minor improvements for code readability and tests"
(SeongJae Park)
- "mm/damon: fix macro arguments and clarify quota goals doc" (Maksym
Shcherba)
- "userfaultfd: merge fs/userfaultfd.c into mm/userfaultfd.c" (Mike
Rapoport)
- "mm/mglru: improve reclaim loop and dirty folio" (Kairui Song and
others)
Clean up and slightly improves MGLRU's reclaim loop and dirty
writeback handling. Large performance improvements are measured

- "memcg: shrink obj_stock_pcp and cache multiple objcgs" (Shakeel
Butt)
Resolve a 68% performance regression caused by NUMA-node cache
thrashing around struct obj_stock_pcp by shrinking its existing
fields and expanding it into a multi-slot array that caches up to
five obj_cgroup pointers per CPU, allowing per-node variants of the
same memcg to coexist within a single 64-byte cache line.

Rename the memory block lookup helper to make the acquired reference
explicit, add memory_block_put() to wrap put_device(), remove
find_memory_block(), and use memory_block_get() as the single block-id
based lookup interface.
This makes it clearer to callers that a successful lookup holds a
reference that must be dropped, reducing the chance of forgetting the
matching put and leaking the memory block device reference.

parse_system_parameter_string
strcpy() is deprecated; use strscpy() instead. Use the return value of
strscpy() instead of calling strlen() again, and ignore any potential
string truncation since both strings are much shorter than the buffer
size SPLPAR_MAXLENGTH.
Change both if conditions to silence the following checkpatch warnings:

Replace sprintf() with sysfs_emit() and sysfs_emit_at() in sysfs show
functions, which are preferred for formatting sysfs output because they
provide safer bounds checking.
While the current code only emits strings that fit easily within
PAGE_SIZE, use sysfs_emit() and sysfs_emit_at() to follow secure coding
best practices.
This is a mechanical cleanup with a few simple edge cases:
- In domains_show(), drop the redundant n < 0 check since neither
sprintf() nor sysfs_emit() return negative values.
- In powercap_show() and psr_show(), also drop the dead ret < 0 checks.
- In ps3_fw_version_show(), normalize the output by adding a terminating
newline as suggested by checkpatch.
- In vio's modalias_show(), replace the deprecated strcpy() [1] followed
by strlen() with sysfs_emit().
Leave validate_show() and the variable-length hv-gpci helpers unchanged
since they already have explicit bounds handling, and converting those
would be more than a mechanical conversion.

The ibm,configure-kernel-dump RTAS call sites in
rtas_fadump_register(), rtas_fadump_unregister(), and
rtas_fadump_invalidate() polled indefinitely while firmware returned
a busy status. A misbehaving or hung firmware could stall these paths
forever, blocking fadump registration at boot or preventing clean
teardown.
Introduce rtas_fadump_call(), a helper that wraps the common
busy-wait pattern shared by all three sites. The helper accumulates
the total delay and returns -ETIMEDOUT if firmware keeps returning a
busy status beyond RTAS_FADUMP_MAX_WAIT_MS (60 seconds). A pr_debug()
message is emitted on each busy iteration to aid diagnosis when the
timeout is hit.

copy_to_user() return bytes_not_copied to the user buffer. If there was
an error writing bytes into the user buffer, i.e. if copy_to_user
returns a non-zero value, then we should simply return -EFAULT from the
->read() call.
Otherwise, in the non-patched version, we may end up mixing
"bytes_not_copied + bytes_copied (HVPIPE_HDR_LEN)" as the return value
to the user in ->read() call
Also let's make sure we clear the hvpipe_status flag, if we have
consumed the hvpipe msg by making the rtas call. ret = -EFAULT means
copy_to_user has failed but that still means that the msg was read from
the hvpipe, hence for both cases, success & -EFAULT, we should clear the
HVPIPE_MSG_AVAILABLE flag in hvpipe_status.

Remove such 3 levels of nesting patterns to check success return values
from function calls.
ret = enable_hvpipe_IRQ()
if (!ret)
ret = set_hvpipe_sys_param(1)
if (!ret)
ret = misc_register()
Instead just bail out to "out*:" labels, in case of any error. This
simplifies the init flow.
While at it let's also fix the following error handling logic:
We have already enabled interrupt sources and enabled hvpipe to received
interrupts, if misc_register() fails, we will destroy the workqueue, but
the HMC might send us a msg via hvpipe which will call, queue work on
the workqueue which might be destroyed.
So instead, let's reverse the order of enabling set_hvpipe_sys_param(1)
and in case of an error let's remove the misc dev by calling
misc_deregister().

commit 6d3789d347a7 ("papr-hvpipe: convert papr_hvpipe_dev_create_handle() to FD_PREPARE()"),
changed the create handle to FD_PREPARE(), but it caused kernel
null-ptr-deref because after call to retain_and_null_ptr(src_info),
src_info is re-used for adding it to the global list.
Getting the following kernel panic in papr_hvpipe_dev_create_handle()
when trying to add src_info to the list.
Kernel attempted to write user page (0) - exploit attempt? (uid: 0)
BUG: Kernel NULL pointer dereference on write at 0x00000000
Faulting instruction address: 0xc0000000001b44a0
Oops: Kernel access of bad area, sig: 11 [#1]
...
Call Trace:
papr_hvpipe_dev_ioctl+0x1f4/0x48c (unreliable)
sys_ioctl+0x528/0x1064
system_call_exception+0x128/0x360
system_call_vectored_common+0x15c/0x2ec
Now, the error handling with FD_PREPARE's file cleanup and __free(kfree) auto
cleanup is getting too convoluted. This is mainly because we need to
ensure only 1 user get the srcID handle. To simplify this, we allocate
prepare the src_info in the beginning and add it to the global list
under a spinlock after checking that no duplicates exist.
This simplify the error handling where if the FD_ADD fails, we can
simply remove the src_info from the list and consume any pending msg in
hvpipe to be cleared, after src_info became visible in the global list.

The hdr variable is allocated on the stack and only hdr.version and
hdr.flags are initialized explicitly. Because the struct papr_hvpipe_hdr
contains reserved padding bytes (reserved[3] and reserved2[40]), these
could leak the uninitialized bytes to userspace after copy_to_user().
This patch fixes that by initializing the whole struct to 0.

H_HTM (Hardware Trace Macro) hypervisor call has capability
to capture SystemMemory Configuration. This information
helps to understand the address mapping for the partitions

Support dumping system memory configuration from Hardware

htmdump modules uses global memory buffers to capture
details like capabilities, status of specified HTM, read the
trace buffer. These are initialized during module init and
hence needs to be freed in module exit.
Patch adds freeing of the memory in module exit. The change
also includes minor clean up for the variable name. The
read call back for the debugfs interface file saves filp->private_data
to local variable name which is same as global variable
name for the memory buffers. Rename these local variable

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".
