epoll_wait() is a valid sleeping reason for real-time tasks because it
uses PI-aware locking, but the rtapp sleep monitor documentation only
discusses clock_nanosleep() and futexes. Document it.
ABORT_SLEEP represents a task restoring TASK_RUNNING before entering
the scheduler. Since the task does not actually block, it becomes
runnable again without a wakeup sequence unsafe for real-time. Document
this behavior.

Add a wakeup monitor to detect a lower-priority task waking up a
higher-priority task.
The rtapp/sleep monitor already detects this. However, that monitor
triggers an error in the context of the wakee task and user only gets
the stacktrace of that task. It is also extremely useful to get the
stacktrace of the waker task, which this monitor offers. In other
words, this monitor complements the rtapp/sleep monitor.

The rtapp/sleep monitor's primary purpose is detecting common mistakes
with user-space real-time design. Monitoring real-time issues with
kernel threads is a bonus.
However, accomodating kernel threads complicates the monitor due to
the edge cases which is seen by the monitor as lower-priority task
waking higher-priority task:
- kthread_stop() wakes up the task in order to stop it.
- The rcu thread and migration thread can be woken by any task.
- The ktimerd thread is woken near the end of irq_exit_rcu(), where
the preempt counter is "broken" and falsely says this is task
context. This requires the monitor to use the hardirq_context flag
instead of the preempt counter.
Beside complicating the monitor, the final case also requires enabling
CONFIG_TRACE_IRQFLAGS (so that "hardirq_context" can be used). This
adds overhead to the kernel even when the monitor is not active. This
may be an obstacle to enabling this monitor in distros' kernels.
Furthermore, kernel threads usually are started before the monitor is
enabled. Consequently, the threads' states (i.o.w. the monitor's
atomic propositions for the threads) are not fully known to the
monitor. As a result, the kernel threads mostly cannot be monitored.
Overall, the downsides of accomodating kernel threads outweights the
benefits. Thus, exclude kernel threads to simplify the monitor.

CLOCK_REALTIME is the only clock that often is misused in real-time
applications. The other clocks either are safe for real-time uses
(CLOCK_TAI, CLOCK_MONOTONIC, CLOCK_BOOTTIME) or are unlikely to be misused
(CLOCK_AUX, CLOCK_PROCESS_CPUTIME_ID).
Update the monitor to only warn about CLOCK_REALTIME.
While at it, update the out-of-sync documentation.

The rtapp/sleep monitor detects real-time tasks which go to sleep in an
real-time-unsafe manner. If this happen, the monitor triggers a trace event
in the sched_wakeup tracepoint's handler.
However, the invoking context of that trace event is not the most
informative, because of the stack trace of that event is the wakeup's code

The sched monitor page was linking to Daniel's website which is now
down. The main purpose of the link was to point to a source for the
models from the original author and that can be found also in his
published paper.
Replace the link with a reference to Daniel's "A thread synchronization
model for the PREEMPT_RT Linux kernel" which can be found online and
includes the models definitions as well as the work behind them (not the
original patches but since they're based on a 5.0 kernel and are mostly
included upstream, there's little value in keeping them in the docs).

The opid monitor validates that wakeup and need_resched events only
occur with interrupts and preemption disabled by following the
preemptirq tracepoints.
As reported in [1], those tracepoints might be inaccurate in some
situations (e.g. NMIs).
Since the monitor doesn't validate other ordering properties, remove the
dependency on preemptirq tracepoints and convert the monitor to a hybrid
automaton to validate the constraint during event handling.
This makes the monitor more robust by also removing the workaround for
interrupts missing the preemption tracepoints, which was working on
PREEMPT_RT only and allows the monitor to be built on kernels without
the preemptirqs tracepoints.

Pull runtime verifier updates from Steven Rostedt:
- Refactor da_monitor to minimize macros
Complete refactor of da_monitor.h to reduce reliance on macros
generating functions. Use generic static functions and uses the
preprocessor only when strictly necessary (e.g. for tracepoint
handlers).
The change essentially relies on functions with generic names (e.g.
da_handle) instead of monitor-specific as well adding the need to
define constant (e.g. MONITOR_NAME, MONITOR_TYPE) before including
the header rather than calling macros that would define functions.
Also adapt monitors and documentation accordingly.
- Cleanup DA code generation scripts
Clean up functions in dot2c removing reimplementations of trivial
library functions (__buff_to_string) and removing some other unused
intermediate steps.
- Annotate functions with types in the rvgen python scripts
- Remove superfluous assignments and cleanup generated code
The rvgen scripts generate a superfluous assignment to 0 for enum
variables and don't add commas to the last elements, which is against
the kernel coding standards. Change the generation process for a
better compliance and slightly simpler logic.
- Remove superfluous declarations from generated code
The monitor container source files contained a declaration and a
definition for the rv_monitor variable. The former is superfluous and
was removed.
- Fix reference to outdated documentation

The tss monitor currently guarantees task switches can happen only while
scheduling, whereas the sncid monitor enforces scheduling occurs with
interrupt disabled.
Replace the monitors with a more comprehensive specification which
implies both but also ensures that:
* each scheduler call disable interrupts to switch
* each task switch happens with interrupts disabled

The 'next' operator is a unary operator. It is defined as: "next time, the
operand must be true".
Support this operator. For RV monitors, "next time" means the next
invocation of ltl_validate().

Monitor synthesis from deterministic automaton and linear temporal logic
have a lot in common. Therefore a single document should describe both.
Change da_monitor_synthesis.rst to monitor_synthesis.rst. LTL monitor
synthesis will be added to this file by a follow-up commit.
This makes the diff far easier to read. If renaming and adding LTL info is
done in a single commit, git wouldn't recognize it as a rename, but a file
removal and a file addition.
While at it, correct the old dot2k commands to the new rvgen commands.

Per task wakeup while not running (wwnr) monitor.
This model is broken, the reason is that a task can be running in the
processor without being set as RUNNABLE. Think about a task about to
sleep:
1: set_current_state(TASK_UNINTERRUPTIBLE);
2: schedule();
And then imagine an IRQ happening in between the lines one and two,
waking the task up. BOOM, the wakeup will happen while the task is
running.
Q: Why do we need this model, so?
A: To test the reactors.

The wakeup event always takes place with preemption disabled because
of the scheduler synchronization. However, because the preempt_count
and its trace event are not atomic with regard to interrupts, some
inconsistencies might happen.
