KVM: s390: Rename kvm-s390.{c,h} to s390.{c,h}
Rename kvm-s390.c to s390.c and kvm-s390.h to s390.h for consistency
with the new directory structure. Update all include statements and
simplify Makefile ccflags from explicit paths to -I$(src).
No functional change.
