Pull xen updates from Juergen Gross:

 - Several small cleanups of various Xen related drivers
   (xen/platform-pci, xen-balloon, xenbus, xen/mcelog)

 - Cleanup for Xen PV-mode related code (includes dropping the Xen
   debugfs code)

 - Drop the additional lazy mmu mode tracking done by Xen specific code
