iommu/vt-d: Remove dmar_readl() and dmar_readq()
dmar_readl() and dmar_readq() do nothing other than expand to the generic
readl() and readq(), and the dmar_read*() wrappers are used inconsistently.
Remove the dmar_read*() wrappers and use readl() and readq() directly.
