There is no hardware which can filter input on the duty cycle, so no
driver implements this. On top of that, LIRC_CAN_SET_REC_DUTY_CYCLE
has the same value as LIRC_CAN_MEASURE_CARRIER (0x02000000).

If the driver reports that the hardware had an overflow, report this to
userspace. It would be nice to know when this happens, and not just get
a long space.

This change has been tested with lircd, ir-ctl, and ir-keytable.

These features have never been implemented by any lirc driver, including
staging or out of tree drivers. The ioctls for these feaures were removed
in commit d55f09abe24b ("[media] lirc.h: remove several unused ioctls").

So, we can safely remove them.

Smatch complains that "rc_proto" comes from the user and it can result
in shift wrapping in ir_raw_encode_scancode()

    drivers/media/rc/rc-ir-raw.c:526 ir_raw_encode_scancode()
    error: undefined (user controlled) shift '1 &lt;&lt; protocol'

This is true, but I reviewed the surrounding code and it appears
harmless. Anyway, let's verify that "rc_proto" is valid as a kernel
hardening measure.

The timestamps in ir-keytable -t output showed that the Xbox DVD
IR dongle decodes scancodes every 64ms. The last scancode of a
longer button press is decodes 64ms after the last-but-one which
indicates the decoder doesn't use a timeout but decodes on the last
edge of the signal.
