This is a simple helper which replaces page_folio(bvec-&gt;bv_page).
Minor improvement in readability, but the real motivation is to reduce
the number of references to bvec-&gt;bv_page so that it can be changed
with less work.

The macros are impossible to follow due to the lack of visual type
information and all the braces.  Replace them with inline helpers to
improve on that.  Because the calling conventions are a bit problematic
with a lot of passing structures by value, all the helpers are marked
as __always_inline so that they are force inlined.

The documentation for bio and bvec_iter refers to a vector named bvl_vec.
This does not exist. Update the documentation comment with correct use.

Also update documentation comments for remaining fields of `bvec_iter` to
improve readability.

The fields of `bvec_iter` is using a mix of tabs and spaces for
indentation. While at it, change them all to tabs, which is most prevalent
in this struct definition.

Similarly to 26064d3e2b4d ("block: fix adding folio to bio"), if
we attempt to add a folio that is larger than 4GB, we'll silently
truncate the offset and len.  Widen the parameters to size_t, assert
that the length is less than 4GB and set the first page that contains
the interesting data rather than the first page of the folio.

Get callers out of poking into bvec internals a bit more.  Not a huge win
right now, but with the proposed new DMA mapping API we might end up with
