<title>generic: make PFN_PHYS explicitly return phys_addr_t</title>

PFN_PHYS, as its name suggests, turns a pfn into a physical address.
However, it is a macro which just operates on its argument without
modifying its type.  pfns are typed unsigned long, but an unsigned
long may not be long enough to hold a physical address (32-bit systems
with more than 32 bits of physcial address).

Make sure we cast to phys_addr_t to return a complete result.

There are four operating modes Xen code may find itself running in:
 - native
 - hvm domain
 - pv dom0
 - pv domU

Clean up predicates for testing for these states to make them more consistent.

This patch is the result of an automatic script that consolidates the
format of all the headers in include/asm-x86/.

The format:

1. No leading underscore. Names with leading underscores are reserved.
2. Pathname components are separated by two underscores. So we can
   distinguish between mm_types.h and mm/types.h.
3. Everything except letters and numbers are turned into single
   underscores.
