All users have now been converted to linux/semaphore.h and we don't need
to keep these files around any longer.

<title>Generic semaphore implementation</title>

Semaphores are no longer performance-critical, so a generic C
implementation is better for maintainability, debuggability and
extensibility.  Thanks to Peter Zijlstra for fixing the lockdep
warning.  Thanks to Harvey Harrison for pointing out that the
unlikely() was unnecessary.

<title>kill DECLARE_MUTEX_LOCKED</title>

DECLARE_MUTEX_LOCKED was used for semaphores used as completions and we've
got rid of them.  Well, except for one in libusual that the maintainer
explicitly wants to keep as semaphore.  So convert that useage to an
explicit sema_init and kill of DECLARE_MUTEX_LOCKED so that new code is
reminded to use a completion.

powerpc: Merge semaphore.h

Adopted the ppc64 version of semaphore.h.  The 32-bit version used
smp_wmb(), but recent updates to atomic.h mean this is no longer required.
The 64-bit version made use of unlikely(), which has been retained in the
combined version.

This patch requires the recent atomic.h patch.
