<title>treewide: use __printf not __attribute__((format(printf,...)))</title>

Standardize the style for compiler based printf format verification.
Standardized the location of __printf too.

Done via script and a little typing.

$ grep -rPl --include=*.[ch] -w "__attribute__" * | \
  grep -vP "^(tools|scripts|include/linux/compiler-gcc.h)" | \
  xargs perl -n -i -e 'local $/; while (&lt;&gt;) { s/\b__attribute__\s*\(\s*\(\s*format\s*\(\s*printf\s*,\s*(.+)\s*,\s*(.+)\s*\)\s*\)\s*\)/__printf($1, $2)/g ; print; }'

<title>ntfs: don't fool kernel-doc</title>

kernel-doc handles macros now (it has for quite some time), so change the
ntfs_debug() macro's kernel-doc to be just before the macro instead of
before a phony function prototype.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
