Pull Xen bugfixes from Konrad Rzeszutek Wilk:
 - Use appropriate macros instead of hand-rolling our own (ARM).
 - Fixes if FB/KBD closed unexpectedly.
 - Fix memory leak in /dev/gntdev ioctl calls.
 - Fix overflow check in xenbus_file_write.
 - Document cleanup.
 - Performance optimization when migrating guests.

map-&gt;kmap_ops allocated in gntdev_alloc_map() wasn't freed by
gntdev_put_map().

Add a gntdev_free_map() helper function to free everything allocated
by gntdev_alloc_map().

This correctly sizes it as 64 bit on ARM but leaves it as unsigned
long on x86 (therefore no intended change on x86).

The long and ulong guest handles are now unused (and a bit dangerous)
so remove them.
