tty: move drivers/serial/ to drivers/tty/serial/

The serial drivers are really just tty drivers, so move them to
drivers/tty/ to make things a bit neater overall.

This is part of the tty/serial driver movement proceedure as proposed by
Arnd Bergmann and approved by everyone involved a number of months ago.

kgdboc,input: Fix regression with keyboard release key and early debugging

The commit 111c182340cd22e238ab1cc6564df336c6ebd7cb (kgdboc: reset
input devices (keyboards) when exiting debugger) introduced a
regression in early debugging such that you get a kernel oops on
continue (with the go command) if you boot a kernel with:

    earlyprintk=vga ekgdboc=kbd kgdbwait

The restore kgdboc_restore_input() routine schedules work for the
purpose of sending key release events for any keys that were in the
depressed state prior to entering the kernel debugger.  A simple fix
to the crash is to not invoke the schedule_work() if the kernel
system_state is anything other than SYSTEM_RUNNING.

kgdboc: reset input devices (keyboards) when exiting debugger

Use the newly exported input_reset_device() call to reset LED state and
mark all keys/buttons as released on all keyboard-like devices when
exiting the debugger.

The one in kgdb.c required the (void * __force) because of the return
code from register_wide_hw_breakpoint looking like:

        return (void __percpu __force *)ERR_PTR(err);

kgdboc: Add call backs to allow kernel mode switching

kgdboc: Add ekgdboc for early use of the kernel debugger

The ekgdboc= differs from kgdboc= in that you can begin debuggin as
soon as the exceptions are setup and the kgdb I/O driver is available,
instead of waiting until the tty subsystem is available.

kgdboc,keyboard: Keyboard driver for kdb with kgdb

This patch adds in the kdb PS/2 keyboard driver.  This was mostly a
direct port from the original kdb where I cleaned up the code against
checkpatch.pl and added the glue to stitch it into kgdb.

This patch also enables early kdb debug via kgdbwait and the keyboard.

All the access to configure kdb using either a serial console or the
keyboard is done via kgdboc.

If you want to use only the keyboard and want to break in early you
would add to your kernel command arguments:

    kgdboc=kbd kgdbwait

If you wanted serial and or the keyboard access you could use:

    kgdboc=kbd,ttyS0

You can also configure kgdboc as a kernel module or at run time with
the sysfs where you can activate and deactivate kgdb.

Turn it on:
    echo kbd,ttyS0 > /sys/module/kgdboc/parameters/kgdboc

Turn it off:
    echo "" > /sys/module/kgdboc/parameters/kgdboc

tty: The big operations rework

- Operations are now a shared const function block as with most other Linux
  objects

- Introduce wrappers for some optional functions to get consistent behaviour

- Wrap put_char which used to be patched by the tty layer

- Document which functions are needed/optional

- Make put_char report success/fail

- Cache the driver->ops pointer in the tty as tty->ops

- Remove various surplus lock calls we no longer need

- Remove proc_write method as noted by Alexey Dobriyan

- Introduce some missing sanity checks where certain driver/ldisc
  combinations would oops as they didn't check needed methods were present
