[S390] move sysinfo.c from drivers/s390 to arch/s390/kernel

All in sysinfo.c is core kernel code and not driver code. So move it
to arch/s390/kernel. Also includes some small cleanups.

to report a relevant service level, e.g. the microcode level of
devices, the service level of the hypervisor, etc.

[S390] KVM preparation: split sysinfo definitions for kvm use

drivers/s390/sysinfo.c uses the store system information intruction to query
the system about information of the machine, the LPAR and additional
hypervisors. KVM has to implement the host part for this instruction.

To avoid code duplication, this patch splits the common definitions from
sysinfo.c into a separate header file include/asm-s390/sysinfo.h for KVM use.

calibrate_delay() must be __cpuinit

calibrate_delay() must be __cpuinit, not __{dev,}init.

I've verified that this is correct for all users.

While doing the latter, I also did the following cleanups:
- remove pointless additional prototypes in C files
- ensure all users #include 

This fixes the following section mismatches with CONFIG_HOTPLUG=n,
CONFIG_HOTPLUG_CPU=y:

[S390] Processor degradation notification.

Generate uevents for all cpus if cpu capability changes. This can
happen e.g. because the cpus are overheating. The cpu capability can
be read via /sys/devices/system/cpu/cpuN/capability.

[S390] Calibrate delay and bogomips.

Preset the bogomips number to the cpu capacity value reported by
store system information in SYSIB 1.2.2. This value is constant
for a particular machine model and can be used to determine
relative performance differences between machines.

[S390] cleanup sysinfo and add system z9 specific extensions.

With System z9 additional fields have been added to the output of the
store system information instruction. This patch adds the new model
information field and the alternate cpu capability fields to the
output of /proc/sysinfo. While we at it clean up the code as well.

[PATCH] s390: cleanup Kconfig

Sanitize some s390 Kconfig options.  We have ARCH_S390, ARCH_S390X,
ARCH_S390_31, 64BIT, S390_SUPPORT and COMPAT.  Replace these 6 options by
S390, 64BIT and COMPAT.
