There is not much point of having a file with a single function in it.
Instead we can just move pci_hp_add_bridge() to drivers/pci/probe.c and
make it available always when PCI core is enabled.

Some PCI functions used to be marked __devinit.  When CONFIG_HOTPLUG was
not set, these functions were discarded after boot.  A few callers of these
__devinit functions were marked __ref to indicate that they could safely
call the __devinit functions even though the callers were not __devinit.

But CONFIG_HOTPLUG and __devinit are now gone, and the need for the __ref
annotations is also gone, so remove them.  Relevant historical commits:

  54b956b90360 Remove __dev* markings from init.h
  a8e4b9c101ae PCI: add generic pci_hp_add_bridge()
  0ab2b57f8db8 PCI: fix section mismatch warning in pci_scan_child_bus
  451124a7cc6c PCI: fix 4x section mismatch warnings

They were implicitly getting it from device.h --&gt; module.h but
we want to clean that up.  So add the minimal header for these

Fix following warning:
WARNING: vmlinux.o(.text+0x47bdb1): Section mismatch in reference from the function pci_scan_child_bus() to the function .devinit.text:pcibios_fixup_bus()

We had plenty of functions that could be annotated __devinit but due to
the former restriction that exported symbols could not be annotated
they were not so.  So annotate these function and fix the references
from the pci/hotplug/* code to silence the resuting warnings.

PCI hotplug were the only user of pci_do_scan_bus()
so moving this function to a separate file that is build
only when we enable CONFIG_HOTPLUG_PCI.
