When using the cfg80211_inform_bss[_width]() functions drivers
cannot currently indicate whether the data was received in a
beacon or probe response. Fix that by passing a new enum that
indicates such (or unknown).

For good measure, use it in ath6kl.

We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to
meet kernel coding style guidelines.  This issue was reported by checkpatch.

Now that the config option is gone, the driver needs to be converted
over to using the dynamic_debug infrastructure, like all other USB
drivers have been already.
