The function pci_unmap_page is obsolete. So it is replaced with
the function dma_unmap_page.

In xmit process, the variables are set many times. In fact,
it is enough for these variables to be set once.
After a long time test, the throughput performance is better
than before.

In preparation for unconditionally passing the struct timer_list pointer to
all timer callbacks, switch to using the new timer_setup() and from_timer()
to pass the timer pointer explicitly.

Since netif_carrier_on() will do nothing if device's
carrier is already on, so it's unnecessary to do
carrier status check.

It's the same for netif_carrier_off().

The ethtool api {get|set}_settings is deprecated.
We move this driver to new api {get|set}_link_ksettings.

As I don't have the hardware, I'd be very pleased if
someone may test this patch.
