<title>pci: use pci_ioremap_bar() in drivers/misc</title>

Use the newly introduced pci_ioremap_bar() function in drivers/misc.
pci_ioremap_bar() just takes a pci device and a bar number, with the goal
of making it really hard to get wrong, while also having a central place
to stick sanity checks.

while we are at it - it's already been zeroed.

The IBM asm driver is using a kobject only for reference counting,
nothing else.  So switch it to use a kref instead, which is all that is
needed, and is much smaller.

get rid of input BIT* duplicate defines

use newly global defined macros for input layer. Also remove includes of
input.h from non-input sources only for BIT macro definiton. Define the
macro temporarily in local manner, all those local definitons will be
removed further in this patchset (to not break bisecting).
BIT macro will be globally defined (1&lt;&lt;x)
