The hpsa driver now has support for all boards the cciss driver
used to support, so this patch removes the cciss driver and
make hpsa an alias to cciss.

Pull SCSI updates from James Bottomley:
 "This update includes the usual round of major driver updates (ncr5380,
  ufs, lpfc, be2iscsi, hisi_sas, storvsc, cxlflash, aacraid,
  megaraid_sas, ...).

  There's also an assortment of minor fixes and the major update of
  switching a bunch of drivers to pci_alloc_irq_vectors from Christoph"

Prepare to mark sensitive kernel structures for randomization by making
sure they're using designated initializers. These were identified during
allyesconfig builds of x86, arm, and arm64, with most initializer fixes
extracted from grsecurity.

Under certain workloads a command may seem to get lost. IOW, the Smart Array
thinks all commands have been completed but we still have commands in our
completion queue. This may lead to system instability, filesystems going
read-only, or even panics depending on the affected filesystem. We add an
extra read to force the write to complete.

Testing shows this extra read avoids the problem.
