<title>module: merge module_alloc() finally</title>

As Christoph Hellwig suggested, module_alloc() actually can be
unified for i386 and x86_64 (of course, also UML).

<title>x86 module: merge the rest functions with macros</title>

Merge the rest functions together, with proper preprocessing directives.
Finally remove module_{32|64}.c.
