ps3_setup_uhc_device() is only called from ps3_setup_ehci_device() and
ps3_setup_ohci_device(), which are both marked __init. Hence replace
the former's __ref marker by __init.

Note that before commit bd721ea73e1f9655 ("treewide: replace obsolete
_refok by __ref"), the function was marked __init_refok, which probably
should have been __init in the first place.

<title>powerpc: Fix PS3 allmodconfig warning</title>

The struct ps3_notification_device in the ps3_probe_thread routine
is too large to be on the stack, causing a warning for an
allmodconfig build with clang.

Change the struct ps3_notification_device from a variable on the stack
to a dynamically allocated variable.

The kernel thread function ps3_probe_thread() invokes the try_to_freeze()
in its loop. But all the kernel threads are non-freezable by default.
So if we want to make a kernel thread to be freezable, we have to invoke
set_freezable() explicitly.

<title>powerpc/ps3: Convert half completion to rcuwait</title>

The PS3 notification interrupt and kthread use a hacked up completion to
communicate. Since we're wanting to change the completion implementation and
this is abuse anyway, replace it with a simple rcuwait since there is only ever
the one waiter.

AFAICT the kthread uses TASK_INTERRUPTIBLE to not increase loadavg, kthreads
cannot receive signals by default and this one doesn't look different. Use
TASK_IDLE instead.

<title>powerpc/ps3: Use struct_size() in kzalloc()</title>

One of the more common cases of allocation size calculations is finding the
size of a structure that has a zero-sized array at the end, along with memory
for some number of elements for that array. For example:

struct foo {
    int stuff;
    void *entry[];
};

instance = kzalloc(sizeof(struct foo) + sizeof(void *) * count, GFP_KERNEL);

Instead of leaving these open-coded and prone to type mistakes, we can now
use the new struct_size() helper:

instance = kzalloc(struct_size(instance, entry, count), GFP_KERNEL);

This code was detected with the help of Coccinelle.

There was only one use of __initdata_refok and __exit_refok

__init_refok was used 46 times against 82 for __ref.

Those definitions are obsolete since commit 312b1485fb50 ("Introduce new
section reference annotations tags: __ref, __refdata, __refconst")

This patch removes the following compatibility definitions and replaces
them treewide.

/* compatibility defines */
#define __init_refok     __ref
#define __initdata_refok __refdata
#define __exit_refok     __ref

I can also provide separate patches if necessary.
(One patch per tree and check in 1 month or 2 to remove old definitions)

Since commit [e58aa3d2: genirq: Run irq handlers with interrupts disabled],
We run all interrupt handlers with interrupts disabled
and we even check and yell when an interrupt handler
