io.c uses open-coded barriers. Update it to use the equivalent but in
macro form.

Sparse reports dereferencing an __iomem pointer. These routines
are clearly low level handlers for IO memory, so force cast away
the __iomem annotation to tell sparse the dereferences are safe.

implement a set of hooks for IO port access to use the LPC
bus via OPAL calls for the first 64K of IO space

All these files were including module.h just for the basic
EXPORT_SYMBOL infrastructure.  We can shift them off to the
export.h header which is a way smaller footprint and thus
realize some compile time gains.

This trivial patch changes memcpy_(to|from)io as to transfer as many
32-bit words as possible in 32-bit accesses (in the current solution,
the last 32-bit word was transferred as 4 byte accesses).

than direct inline asm.  This is part code clean up and partially
because not all PPCs have eieio (book-e has mbar that maps to eieio).

The rework on io.h done for the new hookable accessors made it easier,
so I just finished the work and merged 32 and 64 bits io.h for arch/powerpc.

arch/ppc still uses the old version in asm-ppc, there is just too much gunk
in there that I really can't be bothered trying to cleanup.
