<title>alpha: remove remaining __FUNCTION__ occurrences</title>

__FUNCTION__ is gcc-specific, use __func__

The change in pci-iommu,c should be safe as arena has not been assigned
when we get to this point.

Some were within #if 0 blocks, have changed them and left the blocks
as they appear to be debugging infrastructure.

A #define FN __FUNCTION__ was removed and occurances of FN were replaced
with __func__ as well.

<title>Update for the srm_env driver.</title>

This patch contains a fix for a bug introduced more than a year ago
(not setting *eof) and updates whitespace a bit.

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
