KVM: s390: remove outdated documentation

The old Documentation/s390/kvm.txt file is either
outdated or described in Documentation/virtual/kvm/api.txt.
Let's get rid of it.

qeth: OSA version of SETBRIDGEPORT command

OSA Ethernet hardware is introducing BRIDGEPORT functionality
similar (but not identical) to HiperSockets BRIDGEPORT. This
patch makes HiperSockets BRIDGEPORT related sysfs attributes
and udev events work with OSA hardware too.

s390/docs: Remove section about script debugging from Debugging390.txt

The section about debugging scripting languages has nothing to do with
s390 (and the example is even apparently taken from a i586 host instead),
so let's remove this chapter from the file.

s390/docs: Break long lines in Debugging390.txt

There are a lot of lines that are longer than 80 columns in this file,
rendering it hard to read in a terminal window. This patch fixes most
of these long lines, and while we're at it, also makes some sentences
more readable, e.g. by replacing "&" with "and", adding proper
punctuation, removing superfluous clauses, etc.

Information how to use the GCC pre-processor, objdump, strace, top, etc.
are generic and not specific to the S390 architecture, so we do not need
this information in Debugging390.txt

s390/docs: Fix the documentation of the address spaces

The information about the address spaces was completely outdated, since
the usage of the address spaces changed quite a bit since the early days.
This patch now updates the information about the usage of the address
spaces, mostly by using the description from Heiko's patch "rework uaccess
code - fix locking issues" (457f2180951cdcbfb4657ddcc83b486e93497f56).

s390/zfcpdump: Update documentation in zfcpdump.txt

Do the following changes:

 - Document new /proc/vmcore interface
 - Document partition dump external initramfs with s390-tools-1.24.0
 - Remove initramfs config file because initramfs is now built automatically
   in s390-tools
 - Replace description of kernel config options with "make zfcpdump_defconfig"
 - Some editorial changes

s390/dump: Remove CONFIG_ZFCPDUMP

Currently there are two s390 kernel dump config options "CONFIG_ZFCPDUMP"
and "CONFIG_CRASH_DUMP". In order to keep things simple and because the
"CONFIG_ZFCPDUMP" option already has a dependency to "CONFIG_CRASH_DUMP"
remove the CONFIG_ZFCPDUMP option.
