[S390] cio: Remove cio_msg kernel parameter.

The only sporadically used CIO_DEBUG messages are replaced by ordinary
CIO_MSG_EVENT messages. The CIO_MSG_EVENT messages debug levels are
consolidated.

[S390] Add debug_register_mode() function to debug feature API

The new function supports setting of permissions for the debugfs files
created by the debug feature. In addition to that, the function provides
uid and gid as parameters for future use. Currently only root is allowed
for uid and gid.

[S390] cio: Dump ccw device information in case of timeout.

Information about a ccw device will be dumped in
case of a ccw timeout. This can be enabled with
the kernel parameter ccw_timeout_log.

  advise to issue the channel program from the driver itself.
- Remove some really obsolete and incorrect stuff.
