Convert all text files with s390 documentation to ReST format.

Tried to preserve as much as possible the original document
format. Still, some of the files required some work in order
for it to be visible on both plain text and after converted
to html.

The conversion is actually:
  - add blank lines and identation in order to identify paragraphs;
  - fix tables markups;
  - add some lists markups;
  - mark literal blocks;
  - adjust title markups.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.

The first bit/value table inside the document is very
hard to read and won't fit ReST format. Also, some columns aren't
properly aligned.

Convert it to a nice ascii artwork table with makes it easier to
read as plain text and is compatible with ReST format parser
on Sphinx.

s390 (and the example is even apparently taken from a i586 host instead),

There are a lot of lines that are longer than 80 columns in this file,
rendering it hard to read in a terminal window. This patch fixes most
of these long lines, and while we're at it, also makes some sentences
more readable, e.g. by replacing "&amp;" with "and", adding proper
punctuation, removing superfluous clauses, etc.

The information about the address spaces was completely outdated, since
the usage of the address spaces changed quite a bit since the early days.
This patch now updates the information about the usage of the address
spaces, mostly by using the description from Heiko's patch "rework uaccess
code - fix locking issues" (457f2180951cdcbfb4657ddcc83b486e93497f56).

Remove last traces of our kerntypes patch which was always an addon
patch which never got upstream. Somehow a few bits got upstream
anyway.
Since kerntypes aren't used anymore and lcrash isn't maintained (for
s390 at least) remove the last traces of kerntypes that somehow went
upstream. Also remove the documentation that mentions lcrash.

Below you will find an updated version from the original series bunching all patches into one big patch
updating broken web addresses that are located in Documentation/*
Some of the addresses date as far far back as 1995 etc... so searching became a bit difficult,
the best way to deal with these is to use web.archive.org to locate these addresses that are outdated.
Now there are also some addresses pointing to .spec files some are located, but some(after searching
on the companies site)where still no where to be found. In this case I just changed the address
to the company site this way the users can contact the company and they can locate them for the users.
