Make various changes to the documentation formatting to avoid docs
build errors and otherwise improve the produced output format:

- use bullets for lists
- don't use a '.' at the end of echo commands
- fix indentation

<title>NFSD: Enforce timeout on layout recall and integrate lease manager fencing</title>

When a layout conflict triggers a recall, enforcing a timeout is
necessary to prevent excessive nfsd threads from being blocked in
__break_lease ensuring the server continues servicing incoming
requests efficiently.

This patch introduces a new function to lease_manager_operations:

lm_breaker_timedout: Invoked when a lease recall times out and is
about to be disposed of. This function enables the lease manager
to inform the caller whether the file_lease should remain on the
flc_list or be disposed of.

For the NFSD lease manager, this function now handles layout recall
timeouts. If the layout type supports fencing and the client has not
been fenced, a fence operation is triggered to prevent the client
from accessing the block device.

While the fencing operation is in progress, the conflicting file_lease
remains on the flc_list until fencing is complete. This guarantees
that no other clients can access the file, and the client with
exclusive access is properly blocked before disposal.

The documentation for nfs4_unique_id is out-of-date.  In particular it
claim that when nfs4_unique_id is set, the host name is not used.  since
Commit 55b592933b7d ("NFSv4: Fix nfs4_init_uniform_client_string for net
namespaces") both the unique_id AND the host name are used.

Update the documentation to match the code.

It was an interesting idea but nobody seems to be using it, it's buggy
at this point, and nfs4state.c is already complicated enough without it.
The new nfsd/clients/ code provides some of the same functionality, and
could probably do more if desired.

This feature has been deprecated since 9d60d93198c6 ("Deprecate nfsd
fault injection").

Reduces attack surface on kernel devs opening the links for MITM
as HTTPS traffic is much harder to manipulate.

Deterministic algorithm:
For each file:
  If not .svg:
    For each line:
      If doesn't contain `\bxmlns\b`:
        For each link, `\bhttp://[^# \t\r\n]*(?:\w|/)`:
          If both the HTTP and HTTPS versions
          return 200 OK and serve the same content:
            Replace HTTP with HTTPS.
