While installing SLES-12 (based on v4.4), I found that the installer
will stall for 60+ seconds during LVM disk scan. The root cause was
determined to be the removal of a bound device check in loop_flush()
by commit b5dd2f6047ca ("block: loop: improve performance via blk-mq").
Restoring this check, examining ->lo_state as set by loop_set_fd()
eliminates the bad behavior.

Note that from loop10 onward, the device is not mounted, yet the
stock kernel consumes several orders of magnitude more wall time
than it does for a mounted device.

The following commit:
commit aa4d86163e4e ("block: loop: switch to VFS ITER_BVEC")
replaced __do_lo_send_write(), which used ITER_KVEC iterators, with
lo_write_bvec() which uses ITER_BVEC iterators. In this change, though,
the WRITE flag was lost:
- iov_iter_kvec(&from, ITER_KVEC | WRITE, &kvec, 1, len);
+ iov_iter_bvec(&i, ITER_BVEC, bvec, 1, bvec->bv_len);
This flag is necessary for the DAX case because we make decisions based on
whether or not the iterator is a READ or a WRITE in dax_iomap_actor() and
in dax_iomap_rw().
We end up going through this path in configurations where we combine a PMEM
device with 4k sectors, a loopback device and DAX. The consequence of this
missed flag is that what we intend as a write actually turns into a read in
the DAX code, so no data is ever written.
The very simplest test case is to create a loopback device and try and
write a small string to it, then hexdump a few bytes of the device to see
if the write took. Without this patch you read back all zeros, with this
you read back the string you wrote.
For XFS this causes us to fail or panic during the following xfstests:
xfs/074 xfs/078 xfs/216 xfs/217 xfs/250
For ext4 we have a similar issue where writes never happen, but we don't
currently have any xfstests that use loopback and show this issue.
Fix this by restoring the WRITE flag argument to iov_iter_bvec(). This
causes the xfstests to all pass.

Commit 523e1d399ce0 ("block: make gendisk hold a reference to its queue")
modified add_disk() and disk_release() but did not update any of the
error paths that trigger a put_disk() call after disk->queue has been
assigned. That introduced the following behavior in the pktcdvd driver
if pkt_new_dev() fails:
Kernel BUG at 00000000e98fd882 [verbose debug info unavailable]
Since disk_release() calls blk_put_queue() anyway if disk->queue != NULL,
fix this by removing the blk_cleanup_queue() call from the pkt_setup_dev()
error path.

范龙飞 reports that KASAN can report a use-after-free in __lock_acquire.
The reason is due to insufficient serialization in lo_release(), which
will continue to use the loop device even after it has decremented the
lo_refcnt to zero.
In the meantime, another process can come in, open the loop device
again as it is being shut down. Confusion ensues.

Rather than constructing a local structure instance on the stack, fill
the fields directly on the shared ring, just like other backends do.
Build on the fact that all response structure flavors are actually
identical (the old code did make this assumption too).

the writeback path for cloned images -- we attempt a stat on the parent
object to see if it exists and potentially read it in to call copyup.
GFP_NOIO should be used instead of GFP_KERNEL here.

to the PCIe adapter before the doorbell is triggered. This patch

Since put_disk() triggers a disk_release() call and since that
last function calls blk_put_queue() if disk->queue != NULL, clear
the disk->queue pointer before calling put_disk(). This avoids

If blk_mq_init_queue() returns an error, it gets assigned to
vblk->disk->queue. Then, when we call put_disk(), we end up calling
blk_put_queue() with the ERR_PTR, causing a bad dereference. Fix it by
only assigning to vblk->disk->queue on success.

There is no need to use xen_blkif_get()/xen_blkif_put() in the kthread
of xen-blkback. Thread stopping is synchronous and using the blkif
reference counting in the kthread will avoid to ever let the reference
count drop to zero at the end of an I/O running concurrent to
disconnecting and multiple rings.
Setting ring->xenblkd to NULL after stopping the kthread isn't needed
as the kthread does this already.

The copy_page is optimized memcpy for page-alinged address. If it is
used with non-page aligned address, it can corrupt memory which means
system corruption. With zram, it can happen with
1. 64K architecture
2. partial IO
3. slub debug
Partial IO need to allocate a page and zram allocates it via kmalloc.
With slub debug, kmalloc(PAGE_SIZE) doesn't return page-size aligned
address. And finally, copy_page(mem, cmem) corrupts memory.
So, this patch changes it to memcpy.
Actuaully, we don't need to change zram_bvec_write part because zsmalloc
returns page-aligned address in case of PAGE_SIZE class but it's not
good to rely on the internal of zsmalloc.
Note:
When this patch is merged to stable, clear_page should be fixed, too.
Unfortunately, recent zram removes it by "same page merge" feature so
it's hard to backport this patch to -stable tree.
I will handle it when I receive the mail from stable tree maintainer to
merge this patch to backport.

In zram_rw_page, the logic to get offset is wrong by operator precedence
(i.e., "<<" is higher than "&"). With wrong offset, zram can corrupt
the user's data. This patch fixes it.

Make the "Attempted send on closed socket" error messages generated in
nbd_request_handler() ratelimited.
When the nbd socket is shutdown, the nbd_request_handler() function emits
an error message for every request remaining in its queue. If the queue
is large, this will spam a large amount of messages to the log. There's
no need for a separate error message for each request, so this patch
ratelimits it.
In the specific case this was found, the system was virtual and the error
messages were logged to the serial port, which overwhelmed it.

gcc-6.0 found an ancient bug in the paride driver, which had a
"module_param(verbose, bool, 0);" since before 2.6.12, but actually uses
it to accept '0', '1' or '2' as arguments:

In 2012, Rusty did a cleanup patch that also changed the type of the
variable to 'bool', which introduced what is now a gcc warning.
This changes the type back to 'int' and adapts the module_param() line
instead, so it should work as documented in case anyone ever cares about
running the ancient driver with debugging.

but device node will be created by add_disk() only after successful

As of 5a60e87603c4c533492c515b7f62578189b03c9c, RBD object request
allocations are made via rbd_obj_request_create() with GFP_NOIO.
However, subsequent OSD request allocations in rbd_osd_req_create*()
use GFP_ATOMIC.
With heavy page cache usage (e.g. OSDs running on same host as krbd
client), rbd_osd_req_create() order-1 GFP_ATOMIC allocations have been
observed to fail, where direct reclaim would have allowed GFP_NOIO
allocations to succeed.

When we're using LZ4 multi compression streams for zram swap, we found
out page allocation failure message in system running test. That was
not only once, but a few(2 - 5 times per test). Also, some failure
cases were continually occurring to try allocation order 3.
In order to make parallel compression private data, we should call
kzalloc() with order 2/3 in runtime(lzo/lz4). But if there is no order
2/3 size memory to allocate in that time, page allocation fails. This
patch makes to use vmalloc() as fallback of kmalloc(), this prevents
page alloc failure warning.
After using this, we never found warning message in running test, also
It could reduce process startup latency about 60-120ms in each case.

We can end up allocating a new compression stream with GFP_KERNEL from
within the IO path, which may result is nested (recursive) IO
operations. That can introduce problems if the IO path in question is a
reclaimer, holding some locks that will deadlock nested IOs.
Allocate streams and working memory using GFP_NOIO flag, forbidding
recursive IO and FS operations.
An example:

Commit 4e752f0ab0e8 ("rbd: access snapshot context and mapping size
safely") moved ceph_get_snap_context() out of rbd_img_request_create()
and into rbd_queue_workfn(), adding a ceph_put_snap_context() to the
error path in rbd_queue_workfn(). However, rbd_img_request_create()
consumes a ref on snapc, so calling ceph_put_snap_context() after
a successful rbd_img_request_create() leads to an extra put. Fix it.

Resources are reallocated for requeued commands, so unmap and release
the iod for the failed command.
It's a pretty bad memory leak and causes a kernel hang if you remove a
drive because of a busy dma pool. You'll get messages spewing like this:
nvme 0000:xx:xx.x: dma_pool_destroy prp list 256, ffff880420dec000 busy
and lock up pci and the driver since removal never completes while
holding a lock.

xen-blkfront will crash if the check to talk_to_blkback()
in blkback_changed()(XenbusStateInitWait) returns an error.
The driver data is freed and info is set to NULL. Later during
the close process via talk_to_blkback's call to xenbus_dev_fatal()
the null pointer is passed to and dereference in blkfront_closing.

Mapping an image with a long parent chain (e.g. image foo, whose parent
is bar, whose parent is baz, etc) currently leads to a kernel stack
overflow, due to the following recursion in the reply path:

Limit the parent chain to 16 images, which is ~5K worth of stack. When
the above recursion is eliminated, this limit can be lifted.

Currently we leak parent_spec and trigger a "parent reference
underflow" warning if rbd_dev_create() in rbd_dev_probe_parent() fails.
The problem is we take the !parent out_err branch and that only drops
refcounts; parent_spec that would've been freed had we called
rbd_dev_unparent() remains and triggers rbd_warn() in
rbd_dev_parent_put() - at that point we have parent_spec != NULL and
parent_ref == 0, so counter ends up being -1 after the decrement.
Redo rbd_dev_probe_parent() to fix this.

rbd requires stable pages, as it performs a crc of the page data before
they are send to the OSDs.
But since kernel 3.9 (patch 1d1d1a767206fbe5d4c69493b7e6d2a8d08cc0a0
"mm: only enforce stable page writes if the backing device requires
it") it is not assumed anymore that block devices require stable pages.
This patch sets the necessary flag to get stable pages back for rbd.
In a ceph installation that provides multiple ext4 formatted rbd
devices "bad crc" messages appeared regularly (ca 1 message every 1-2
minutes on every OSD that provided the data for the rbd) in the
OSD-logs before this patch. After this patch this messages are pretty
much gone (only ca 1-2 / month / OSD).

If rbd_dev_image_probe() in rbd_dev_probe_parent() fails, header_name
is freed twice: once in rbd_dev_probe_parent() and then in its caller
rbd_dev_image_probe() (rbd_dev_image_probe() is called recursively to
handle parent images).
rbd_dev_probe_parent() is responsible for probing the parent, so it
shouldn't muck with clone's fields.

zcomp_create() verifies the success of zcomp_strm_{multi,single}_create()
through comp->stream, which can potentially be pointing to memory that
was freed if these functions returned an error.
While at it, replace a 'ERR_PTR(-ENOMEM)' by a more generic
'ERR_PTR(error)' as in the future zcomp_strm_{multi,siggle}_create()
could return other error codes. Function documentation updated
accordingly.

We should consider info->feature_persistent when adding indirect page to list
info->indirect_pages, else the BUG_ON() in blkif_free() would be triggered.
When we are using persistent grants the indirect_pages list
should always be empty because blkfront has pre-allocated enough
persistent pages to fill all requests on the ring.

For write/discard obj_requests that involved a copyup method call, the
opcode of the first op is CEPH_OSD_OP_CALL and the ->callback is
rbd_img_obj_copyup_callback(). The latter frees copyup pages, sets
->xferred and delegates to rbd_img_obj_callback(), the "normal" image
object callback, for reporting to block layer and putting refs.
rbd_osd_req_callback() however treats CEPH_OSD_OP_CALL as a trivial op,
which means obj_request is marked done in rbd_osd_trivial_callback(),
*before* ->callback is invoked and rbd_img_obj_copyup_callback() has
a chance to run. Marking obj_request done essentially means giving
rbd_img_obj_callback() a license to end it at any moment, so if another
obj_request from the same img_request is being completed concurrently,
rbd_img_obj_end_request() may very well be called on such prematurally
marked done request:

Calling rbd_img_obj_end_request() on such a request leads to trouble,
in particular because its ->xfferred is 0. We report 0 to the block
layer with blk_update_request(), get back 1 for "this request has more
data in flight" and then trip on
rbd_assert(more ^ (which == img_request->obj_request_count));
with rhs (which == ...) being 1 because rbd_img_obj_end_request() has
been called for both requests and lhs (more) being 1 because we haven't
got a chance to set ->xfferred in rbd_img_obj_copyup_callback() yet.
To fix this, leverage that rbd wants to call class methods in only two
cases: one is a generic method call wrapper (obj_request is standalone)
and the other is a copyup (obj_request is part of an img_request). So
make a dedicated handler for CEPH_OSD_OP_CALL and directly invoke
rbd_img_obj_copyup_callback() from it if obj_request is part of an
img_request, similar to how CEPH_OSD_OP_READ handler invokes
rbd_img_obj_request_read_callback().
Since rbd_img_obj_copyup_callback() is now being called from the OSD
request callback (only), it is renamed to rbd_osd_copyup_callback().

rbd_obj_request_create() is called on the main I/O path, so we need to
use GFP_NOIO to make sure allocation doesn't blow back on us. Not all
callers need this, but I'm still hardcoding the flag inside rather than
making it a parameter because a) this is going to stable, and b) those
callers shouldn't really use rbd_obj_request_create() and will be fixed
in the future.
More memory allocation fixes will follow.

If there are too many pending per work I/O, too many
high priority work thread can be generated so that
system performance can be effected.
This patch limits the max_active parameter of workqueue as 16.
This patch fixes Fedora 22 live booting performance
regression when it is booted from squashfs over dm
based on loop, and looks the following reasons are
related with the problem:
- not like other filesyststems(such as ext4), squashfs
is a bit special, and I observed that increasing I/O jobs
to access file in squashfs only improve I/O performance a
little, but it can make big difference for ext4
- nested loop: both squashfs.img and ext3fs.img are mounted
as loop block, and ext3fs.img is inside the squashfs
- during booting, lots of tasks may run concurrently

If there is dependency among multiple work items used
during memory reclaim, they should be queued to separate
wq each with WQ_MEM_RECLAIM.
Loop devices can be stacked, so we have to convert to per-device
workqueue. One example is Fedora live CD.

A recent change to the ioctl handling caused a new harmless
warning in the NVMe driver on all 32-bit machines:

If userspace asks for an INQUIRY buffer smaller than 36 bytes, the SCSI
translation layer will happily write past the end of the INQUIRY buffer
allocation.
This is fairly easily reproducible by running the libiscsi test
suite and then starting an xfstests run.

- SWIOTLB on ARM now uses frames <4 GiB (if available) so device only
capable of 32-bit DMA work.
