[ Upstream commit 302a1f674c00dd5581ab8e493ef44767c5101aab ]

This attemps to fix possible UAFs caused by struct mgmt_pending being
freed while still being processed like in the following trace, in order
to fix mgmt_pending_valid is introduce and use to check if the
mgmt_pending hasn't been removed from the pending list, on the complete
callbacks it is used to check and in addtion remove the cmd from the list
while holding mgmt_pending_lock to avoid TOCTOU problems since if the cmd
is left on the list it can still be accessed and freed.

[ Upstream commit 9e622804d57e2d08f0271200606bd1270f75126f ]

This fixes the following UFA in hci_acl_create_conn_sync where a
connection still pending is command submission (conn->state == BT_OPEN)
maybe freed, also since this also can happen with the likes of
hci_le_create_conn_sync fix it as well:

[ Upstream commit 9e622804d57e2d08f0271200606bd1270f75126f ]

This fixes the following UFA in hci_acl_create_conn_sync where a
connection still pending is command submission (conn->state == BT_OPEN)
maybe freed, also since this also can happen with the likes of
hci_le_create_conn_sync fix it as well:

Difference between sock_i_uid() and sk_uid() is that
after sock_orphan(), sock_i_uid() returns GLOBAL_ROOT_UID
while sk_uid() returns the last cached sk->sk_uid value.

None of sock_i_uid() callers care about this.

Use sk_uid() which is much faster and inlined.

Note that diag/dump users are calling sock_i_ino() and
can not see the full benefit yet.

[ Upstream commit 862c628108562d8c7a516a900034823b381d3cba ]

syzbot reported the splat below without a repro.

In the splat, a single thread calling bt_accept_dequeue() freed sk
and touched it after that.

The root cause would be the racy l2cap_sock_cleanup_listen() call
added by the cited commit.

bt_accept_dequeue() is called under lock_sock() except for
l2cap_sock_release().

Two threads could see the same socket during the list iteration
in bt_accept_dequeue():

  CPU1                        CPU2 (close())
  ----                        ----
  sock_hold(sk)               sock_hold(sk);

                              lock_sock(sk)
                              sock_put(sk)
                              bt_accept_unlink(sk)
                                sock_put(sk)        <-- last refcnt
                              bt_accept_unlink(sk)  <-- UAF

Depending on the timing, the other thread could show up in the
"Freed by task" part.

Let's call l2cap_sock_cleanup_listen() under lock_sock() in
l2cap_sock_release().

[ Upstream commit de5d7d3f27ddd4046736f558a40e252ddda82013 ]

list_empty(&hdev->adv_instances) is always true during startup,
so an advertising instance is added by default.
