fat: add ratelimit to fat*_ent_bread()

fat*_ent_bread() can be the cause of too many report on I/O error path.
So use fat_msg_ratelimit() instead.

FAT: use io_schedule_timeout() instead of congestion_wait()

congestion_wait() in this context is just a sleep - block devices do not
support congestion signalling any more.

The goal for this wait, which was introduced in commit ae78bf9c4f5f
("[PATCH] add -o flush for fat") is to wait for any recently written
data to get to storage.  We currently have no direct mechanism to do
this, so a simple wait that behaves identically to the current
congestion_wait() is the best we can do.

This is a step towards removing congestion_wait()

Add some basic sanity-check tests for the fat_checksum() function and
the fat_time_unix2fat() and fat_time_fat2unix() functions. These unit
tests verify these functions return correct output for a number of test
inputs.

These tests were inspired by -- and serve a similar purpose to -- the
timestamp parsing KUnit tests in ext4[1].

Note that, unlike fat_time_unix2fat, fat_time_fat2unix wasn't previously
exported, so this patch exports it as well. This is required for the
case where we're building the fat and fat_test as modules.

Fixed minor checkpatch coding style errors and typos in commit log:

block: move the bdi from the request_queue to the gendisk

The backing device information only makes sense for file system I/O,
and thus belongs into the gendisk and not the lower level request_queue
structure.  Move it there.
