The direction of the pipe argument must match the request-type direction
bit or control requests may fail depending on the host-controller-driver
implementation.

Fix the three requests which erroneously used usb_rcvctrlpipe().

Check for NULL port data in the modem- and line-status handlers to avoid
dereferencing a NULL pointer in the unlikely case where a port device
isn't bound to a driver (e.g. after an allocation failure on port
probe).

Note that the other (stubbed) event handlers qt2_process_xmit_empty()
and qt2_process_flush() would need similar sanity checks in case they
are ever implemented.

Make sure to detect short control-message transfers when fetching
modem and line state in open and when retrieving registers.

This specifically makes sure that an errno is returned to user space on
errors in TIOCMGET instead of a zero bitmask.

Also drop the unused getdevice function which also lacked appropriate
error handling.

The interface read URB is submitted in attach, but was only unlinked by
the driver at disconnect.

In case of a late probe error (e.g. due to failed minor allocation),
disconnect is never called and we would end up with active URBs for an
unbound interface. This in turn could lead to deallocated memory being
dereferenced in the completion callback.

None of these files are actually using any __init type directives
and hence don't need to include &lt;linux/init.h&gt;.  Most are just a
left over from __devinit and __cpuinit removal, or simply due to
code getting copied from one driver to the next.
