The PHY configuration is stored in an opaque "config" field, but when
allocating the structure, its proper size needs to be known. In the case
of UTMI, the proper structure is tegra_utmip_config of which a local
variable already exists, so we can use that to obtain the size from.

Fixes the following warning from the sparse checker:

	drivers/usb/phy/phy-tegra-usb.c:882:17: warning: expression using sizeof(void)

Commit 193ab2a60700 ("usb: gadget: allow multiple gadgets to be built")
apparently required that checks for CONFIG_USB_GADGET_OMAP would be
replaced with checks for CONFIG_USB_OMAP. Do so now for the remaining
checks for CONFIG_USB_GADGET_OMAP, even though these checks have
basically been broken since v3.1.

And, since we're touching this code, use the IS_ENABLED() macro, so
things will now (hopefully) also work if USB_OMAP is modular.

Move probe out of __init section and don't use platform_driver_probe
which cannot be used with deferred probing.

Since commit e9354576 ("gpiolib: Defer failed gpio requests by default")
and 04bf3011 ("regulator: Support driver probe deferral") this driver
might return -EPROBE_DEFER if a gpio_request or regulator_get fails.

Use the wrapper functions for getting and setting the driver data using
platform_device instead of using dev_{get,set}_drvdata() with &amp;of-&gt;dev,
so we can directly pass a struct platform_device.

Commit 94ae9843 (usb: phy: rename all phy drivers to phy-$name-usb.c)
renamed drivers/usb/phy/otg_fsm.h to drivers/usb/phy/phy-fsm-usb.h
but changed drivers/usb/phy/phy-fsm-usb.c to include not existing
"phy-otg-fsm.h" instead of new "phy-fsm-usb.h". This breaks building:
  ...
  drivers/usb/phy/phy-fsm-usb.c:32:25: fatal error: phy-otg-fsm.h: No such file or directory
  compilation terminated.
  make[3]: *** [drivers/usb/phy/phy-fsm-usb.o] Error 1

This commit also missed to modify drivers/usb/phy/phy-fsl-usb.h
to include new "phy-fsm-usb.h" instead of "otg_fsm.h" resulting
in another build breakage:
  ...
  In file included from drivers/usb/phy/phy-fsl-usb.c:46:0:
  drivers/usb/phy/phy-fsl-usb.h:18:21: fatal error: otg_fsm.h: No such file or directory
  compilation terminated.

Fix both issues.
