alloc failures already get standardized OOM
messages and a dump_stack.

For the affected mallocs around these OOM messages:

Converted kzallocs with multiplies to kcalloc.
Converted kmallocs with multiplies to kmalloc_array.
Converted a kmalloc/strlen/strncpy to kstrdup.
Moved a spin_lock below a removed OOM message and
removed a now unnecessary spin_unlock.
Neatened alignment and whitespace.

the i and j are used in for loop and they assigned to zeros in the
for loop, so no need to assign them to zeros again.

copy_from_user() returns the number of bytes remaining to be copied,
and it fails if user pointer is invalid, so return -EFAULT
so that the user gets a FAULT.

this driver duplicates the module_pci_driver code, so remove the
duplicated code and use module_pci_driver and also remove the
