If logout response is not received and ->ep_disconnect() is called then close tcp conn by RST instead of FIN to cleanup conn resources immediately. It was not possible to enable both T10 PI and TPGS because they share the same byte in the INQUIRY response. Logically OR the TPGS value instead of using assignment. Hung task timeouts can result if a qlogic board breaks unexpectedly
while running I/O. These tasks become hung because command srb reference
counts are not going to zero, hence the affected srbs and commands do
not get freed. This fix accounts for this extra reference in the srbs in
the case of a board failure.

Functions lpfc_nlp_set_state() and lpfc_issue_els_prli() dereference
