sd: Disable support for 256 byte/sector disks

256 bytes per sector support has been broken since 2.6.X,
and no-one stepped up to fix this.
So disable support for it.

storvsc: Set the SRB flags correctly when no data transfer is needed

Set the SRB flags correctly when there is no data transfer.  Without this
change some IHV drivers will fail valid commands such as TEST_UNIT_READY.

qla2xxx: remove redundant declaration in 'qla_gbl.h'

Remove 2 redundant extern inline functions: qla8044_set_qsnt_ready() and
qla8044_need_reset_handler(). At present, within upstream next kernel
source code, they are only used within "drivers/scsi/qla2xxx/qla_nx2.c".

hpsa: turn off interrupts when kdump starts

Sometimes when the card is restarted it may cause -
"irq 16: nobody cared (try booting with the "irqpoll" option)"
that is likely caused so, that the card, after the hard reset
finishes, pulls on the irq. Disabling the ints before or after
the hpsa_kdump_hard_reset_controller fixes it.

At this point we can't know in which state the card is,
so using SA5_INTR_OFF + SA5_REPLY_INTR_MASK_OFFSET defines directly,
instead of the function the drivers provides, seems to be apropriate.

hpsa: refine the pci enable/disable handling

When a second(kdump) kernel starts and the hard reset method is used
the driver calls pci_disable_device without previously enabling it,
so the kernel shows a warning -

This patch fixes it, in addition to this I tried to balance also some other pairs
of enable/disable device in the driver.
Unfortunately I wasn't able to verify the functionality for the case of a sw reset,
because of a lack of proper hw.

count are valid after that point.  Also remove the dma mapping helpers

mvsas: fix panic on expander attached SATA devices

mvsas is giving a General protection fault when it encounters an expander
attached ATA device.  Analysis of mvs_task_prep_ata() shows that the driver is
assuming all ATA devices are locally attached and obtaining the phy mask by
indexing the local phy table (in the HBA structure) with the phy id.  Since
expanders have many more phys than the HBA, this is causing the index into the
HBA phy table to overflow and returning rubbish as the pointer.

mvs_task_prep_ssp() instead does the phy mask using the port properties.
Mirror this in mvs_task_prep_ata() to fix the panic.
