On CoCo VMs without confidential VMBus, the netvsc send and receive buffers
must be made host-visible by decrypting them. These buffers are vmalloc'ed,
but set_memory_decrypted()/encrypted() do not work on vmalloc'ed memory.
This use case is (so far) unique to netvsc, so solve it locally rather than
changing the set_memory() or allocation APIs.

Use vmbus_alloc_buffer() to allocate the send and receive buffers, which
will make them host-visible. Store the list of memory chunks in the
netvsc_device struct so they can be individually freed later. Use
vmbus_establish_gpadl_caller_decrypted() so there is no attempt to decrypt
the virtual address.

Appropriately free the buffers with vmbus_free_buffer(). Because vunmap()
and set_memory_encrypted() must run in process context, replace the
rcu_head/call_rcu() pair used to defer free_netvsc_device() with
rcu_work/queue_rcu_work(). This also fixes a small race condition where the
buffers may be accessed while being re-encrypted by moving the
re-encryption after the RCU grace period.

netvsc_copy_to_send_buf() copies page buffer entries into the VMBus
send buffer using phys_to_virt() on the entry PFN. Entries for the
RNDIS header and the skb linear data come from kmalloc'd memory and
are always in the kernel direct map, but entries for skb fragments
reference page cache or user pages, which on 32-bit x86 with
CONFIG_HIGHMEM=y can live above the LOWMEM boundary. For such a page
phys_to_virt() returns an address outside the direct map and the
subsequent memcpy() faults on the transmit softirq path, which is
fatal.

Map the pages with kmap_local_page() instead, handling two properties
of the page buffer entries:

 - pb[i].pfn is a Hyper-V PFN at HV_HYP_PAGE_SIZE (4K) granularity,
   not a native PFN. Reconstruct the physical address first and derive
   the native page from it, so the mapping stays correct where
   PAGE_SIZE &gt; HV_HYP_PAGE_SIZE (e.g. arm64 with 64K pages).

 - Since commit 41a6328b2c55 ("hv_netvsc: Preserve contiguous PFN
   grouping in the page buffer array"), an entry describes a full
   physically contiguous fragment and pb[i].len can exceed PAGE_SIZE,
   while kmap_local_page() maps a single page. Copy page by page,
   splitting at native page boundaries.

The copy path only handles packets smaller than the send section size
(6144 bytes by default); larger packets take the cp_partial path where
only the RNDIS header is copied. So entries here are bounded by the
section size and a copy is split at most once on 4K-page systems. On
!CONFIG_HIGHMEM configs kmap_local_page() folds to page_address() and
no mapping work is added.

This converts some of the visually simpler cases that have been split
over multiple lines.  I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.

Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script.  I probably had made it a bit _too_ trivial.

So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.

The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This was done entirely with mindless brute force, using

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:

Single allocations:	kmalloc(sizeof(TYPE), ...)
are replaced with:	kmalloc_obj(TYPE, ...)

Array allocations:	kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with:	kmalloc_objs(TYPE, COUNT, ...)

Flex array allocations:	kmalloc(struct_size(PTR, FAM, COUNT), ...)

RSS configuration requires a valid RX indirection table. When the device
reports a single receive queue, rndis_filter_device_add() does not
allocate an indirection table, accepting RSS hash key updates in this
state leads to a hang.

Fix this by gating netvsc_set_rxfh() on ndc-&gt;rx_table_sz and return
-EOPNOTSUPP when the table is absent. This aligns set_rxfh with the device
capabilities and prevents incorrect behavior.

Convert the hyperv netvsc driver to use the new .get_rx_ring_count
ethtool operation instead of implementing .get_rxnfc solely for handling
ETHTOOL_GRXRINGS command. This simplifies the code by replacing the
switch statement with a direct return of the queue count.

The new callback provides the same functionality in a more direct way,
following the ongoing ethtool API modernization.

At present VMBus driver is hinged off of CONFIG_HYPERV which entails
lot of builtin code and encompasses too much. It's not always clear
what depends on builtin hv code and what depends on VMBus. Setting
CONFIG_HYPERV as a module and fudging the Makefile to switch to builtin
adds even more confusion. VMBus is an independent module and should have
its own config option. Also, there are scenarios like baremetal dom0/root
where support is built in with CONFIG_HYPERV but without VMBus. Lastly,
there are more features coming down that use CONFIG_HYPERV and add more
dependencies on it.

So, create a fine grained HYPERV_VMBUS option and update Kconfigs for
dependency on VMBus.
