The ehea driver oopses during memory hotplug if the ports are not
up. A simple testcase:

To fix this, initialise the waitqueues during port probe instead

Currently EHEA reports to ethtool as supporting 10M, 100M, 1G and
10G and connected to FIBRE independent of the hardware configuration.
However, when connected to FIBRE the only supported speed is 10G
full-duplex, and the other speeds and modes are only supported
when connected to twisted pair.

Currently the skb array is not fully allocated, and the allocation
is done as it's requested, which is not the expected way.

This patch just allocate the full skb array at driver initialization.
Also, this patch increases ehea version to 107.

Remove ehea_error, ehea_info and ehea_debug macros.
Use pr_fmt, pr_&lt;level&gt;, netdev_&lt;level&gt; and netif_&lt;level&gt; as appropriate.
Fix messages to use trailing "\n", some messages had an extra one
as the old ehea_&lt;level&gt; macros added a trailing "\n".
Coalesced long format strings.

Directly cancel port-&gt;reset_task from ehea_shutdown_single_port()
instead.  As this cancels the work for each port on driver detach,
flushing system_wq from ehea_remove() or ehea_module_exit() is no
longer necessary.
