Previously when doing format enumeration, it was returning all
 formats supported by driver, even if they're not supported by hw.
Add missing check for fmt_ver_flag, so it'll be fixed and only those
 supported by hw will be returned. Similar thing is already done
 in s5p_jpeg_find_format.

It was found by using v4l2-compliance tool and checking result
 of VIDIOC_ENUM_FMT/FRAMESIZES/FRAMEINTERVALS test
and using v4l2-ctl to get list of all supported formats.

Tested on s5pv210-galaxys (Samsung i9000 phone).

The capture DV timings capabilities allowed for a minimum width and
height of 0. So passing a timings struct with 0 values is allowed
and will later cause a division by zero.

Ensure that the width and height must be &gt;= 16 to avoid this.

When the subdevice doesn't provide s_power core ops callback, the
v4l2_subdev_call for s_power returns -ENOIOCTLCMD.  If the subdevice
doesn't have the special handling for its power saving mode, the s_power
isn't required.  So -ENOIOCTLCMD from the v4l2_subdev_call should be
ignored.

The OMAP3 ISP driver manages its MMU mappings through the IOMMU-aware
ARM DMA backend. The current code creates a dma_iommu_mapping and
attaches this to the ISP device, but never detaches the mapping in
either the probe failure paths or the driver remove path resulting
in an unbalanced mapping refcount and a memory leak. Fix this properly.

While experimenting with older compiler versions, I ran
into a warning that no longer shows up on gcc-4.8 or newer:

drivers/media/platform/s3c-camif/camif-capture.c: In function '__camif_subdev_try_format':
drivers/media/platform/s3c-camif/camif-capture.c:1265:25: error: array subscript is below array bounds

This is an off-by-one bug, leading to an access before the start of the
array, while newer compilers silently assume this undefined behavior
cannot happen and leave the loop at index 0 if no other entry matches.

As Sylvester explains, we actually need to ensure that the
value is within the range, so this reworks the loop to be
easier to parse correctly, and an additional check to fall
back on the first format value for any unexpected input.

I found an existing gcc bug for it and added a reduced version
of the function there.

In the case of semi planar formats cb and cr are in the same plane
in memory, meaning that will be set to 'cb' whatever the format is,
and whatever the (packed) order of those components are.
