This driver is for fairly obscure hardware, and has only seen random
drive-by changes after the maintainer stopped working on it in 2005
(about a year and a half after it was introduced).  It has some
"interesting" block layer interactions, so let's just drop it unless
anyone complains.

Minh Yuan reported a concurrency use-after-free issue in the floppy code
between raw_cmd_ioctl and seek_interrupt.

[ It turns out this has been around, and that others have reported the
  KASAN splats over the years, but Minh Yuan had a reproducer for it and
  so gets primary credit for reporting it for this fix   - Linus ]

The problem is, this driver tends to break very easily and nowadays,
nobody is expected to use FDRAWCMD anyway since it was used to
manipulate non-standard formats.  The risk of breaking the driver is
higher than the risk presented by this race, and accessing the device
requires privileges anyway.

Let's just add a config option to completely disable this ioctl and
leave it disabled by default.  Distros shouldn't use it, and only those
running on antique hardware might need to enable it.

Support for cryptoloop has been officially marked broken and deprecated
in favor of dm-crypt (which supports the same broken algorithms if
needed) in Linux 2.6.4 (released in March 2004), and support for it has
been entirely removed from losetup in util-linux 2.23 (released in April
2013).  Add a warning and a deprecation schedule.

Since commit 84af7a6194e4 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.

This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.

There are a variety of indentation styles found.

  a) 4 spaces + '---help---'
  b) 7 spaces + '---help---'
  c) 8 spaces + '---help---'
  d) 1 space + 1 tab + '---help---'
  e) 1 tab + '---help---'    (correct indentation)
  f) 1 tab + 1 space + '---help---'
  g) 1 tab + 2 spaces + '---help---'

In order to convert all of them to 1 tab + 'help', I ran the
following commend:

  $ find . -name 'Kconfig*' | xargs sed -i 's/^[[:space:]]*---help---/\thelp/'

Since the need for a special flag to support SCSI passthrough on a
block device was added in May 2017 the SCSI passthrough support in
virtio-blk has been disabled.  It has always been a bad idea
(just ask the original author..) and we have virtio-scsi for proper
passthrough.  The feature also never made it into the virtio 1.0
or later specifications.

Rename the blockdev documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.

The drbd sub-directory contains some graphs and data flows.
Add those too to the documentation.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.
