This is the 2nd part of fix for kernel bugzilla 40002:
    "IRQ 0 assigned to VGA"

The root cause is the buggy FW, whose ACPI tables assign the GSI 16
to 2 irqs 0 and 16(VGA), and the VGA is the right owner of GSI 16.
So add a quirk to ignore the irq0 overriding GSI 16 for the
FUJITSU SIEMENS AMILO PRO V2030 platform will solve this issue.

This patch iterates through all possible cpus, in case we issue a system reset and CPUs in an offline state call in. Perhaps the old code was trying to handle CPUs that were in the partition but were never started (eg kexec into a kernel with an nr_cpus= boot option). They are going to die way before we get into xmon since we haven't set any kernel state up for them. We have a bug report where the kernel hits a warning in the cpumask
code:
