Naming interrupt domains host is confusing at best and the irqdomain code
uses both domain and host inconsistently.

Therefore rename irq_set_default_host() to irq_set_default_domain().

The following line appears at boot:

	IPIC (128 IRQ sources) at (ptrval)

This is pointless so remove the printing of the virtual address and
replace it by matching physical address.

Don't inherit headers "by chances" from asm/prom.h, asm/mpc52xx.h,
asm/pci.h etc...

Include the needed headers, and remove asm/prom.h when it was
needed exclusively for pulling necessary headers.

Some files functions in 'arch/powerpc/sysdev' are deserving of an `__init`
macro attribute. These functions are only called by other initialization
functions and therefore should inherit the attribute.
Also, change function declarations in header files to include `__init`.

ipic_get_mcp_status() is used by targets implementing NMI
watchdog in target specific machine check handler in order
to known whether a machine check results from a watchdog
NMI reset.

In case of very early machine check, primary_ipic pointer
might not have been set yet, so ipic_get_mcp_status() needs
to check it for nullity before using it.

External IRQ0 (index 48) has the same capabilities as the other IRQ1-7
and is handled by the same register IPIC_SEPNR. When this register is
not specified for "ack" in "ipic_info", you cannot configure this IRQ
as IRQ_TYPE_EDGE_FALLING. This oversight was probably due to the
non-contiguous hwirq numbering of IRQ0 in the IPIC.
