MIPS: KVM: Uninit VCPU in vcpu_create error path

If either of the memory allocations in kvm_arch_vcpu_create() fail, the
vcpu which has been allocated and kvm_vcpu_init'd doesn't get uninit'd
in the error handling path. Add a call to kvm_vcpu_uninit() to fix this.

MIPS: KVM: Fix CACHE immediate offset sign extension

The immediate field of the CACHE instruction is signed, so ensure that
it gets sign extended by casting it to an int16_t rather than just
masking the low 16 bits.

MIPS: KVM: Fix ASID restoration logic

ASID restoration on guest resume should determine the guest execution
mode based on the guest Status register rather than bit 30 of the guest
PC.

Fix the two places in locore.S that do this, loading the guest status
from the cop0 area. Note, this assembly is specific to the trap &
emulate implementation of KVM, so it doesn't need to check the
supervisor bit as that mode is not implemented in the guest.

MIPS: ath79: Fix the DDR control initialization on ar71xx and ar934x

The DDR control initialization needs to know the SoC type, however
ath79_detect_sys_type() was called after ath79_ddr_ctrl_init().
Reverse the order to fix the DDR control initialization on ar71xx and
ar934x.

Add helper macro builtin_mips_cdmm_driver() for builtin CDMM drivers
that don't do anything special in init and have no exit. The
module_mips_cdmm_driver() helper isn't really appropriate for drivers

MIPS: lantiq: add clk_round_rate()

This adds a basic implementation of clk_round_rate()
The clk_round_rate() function is called by multiple drivers and
subsystems now and the lantiq clk driver is supposed to export this,
but doesn't do so, this causes linking problems like this one:
ERROR: "clk_round_rate" [drivers/media/v4l2-core/videodev.ko] undefined!

MIPS: CPS: #ifdef on CONFIG_MIPS_MT_SMP rather than CONFIG_MIPS_MT

The CONFIG_MIPS_MT symbol can be selected by CONFIG_MIPS_VPE_LOADER in
addition to CONFIG_MIPS_MT_SMP. We only want MT code in the CPS SMP boot
vector if we're using MT for SMP. Thus switch the config symbol we ifdef
against to CONFIG_MIPS_MT_SMP.

MIPS: CPS: Don't include MT code in non-MT kernels.

The MT-specific code in mips_cps_boot_vpes can safely be omitted from
kernels which don't support MT, with the default VPE==0 case being used
as it would be after the has_mt (Config3.MT) check failed at runtime.
Discarding the code entirely will save us a few bytes & allow cleaner
handling of MT ASE instructions by later patches.

MIPS: CPS: Stop dangling delay slot from has_mt.

The has_mt macro ended with a branch, leaving its callers with a delay
slot that would be executed if Config3.MT is not set. However it would
not be executed if Config3 (or earlier Config registers) don't exist
which makes it somewhat inconsistent at best. Fill the delay slot in the
macro & fix the mips_cps_boot_vpes caller appropriately.

MIPS: dma-default: Fix 32-bit fall back to GFP_DMA

If there is a DMA zone (usually 24bit = 16MB I believe), but no DMA32
zone, as is the case for some 32-bit kernels, then massage_gfp_flags()
will cause DMA memory allocated for devices with a 32..63-bit
coherent_dma_mask to fall back to using __GFP_DMA, even though there may
only be 32-bits of physical address available anyway.

Correct that case to compare against a mask the size of phys_addr_t
instead of always using a 64-bit mask.
